We invite all students in year 7 through to year 13 to take part in keratoconus screening. Regular eye screening can allow us to identify if this disease is present early and significantly lower the risk of sight loss.

We would then contact you again in 3 years’ time to see if your circumstances have changed and ask if you would like to rejoin the screening programme.

If you do not sign and send the completed form back, we will continue to invite you for screening as usual.
